Thank you for your feedback. During medical evaluations, providers use their clinical judgment to determine the qualifying condition that best aligns with the information discussed during the appointment and the patient’s medical history. While patient input is important and always considered, the final certification decision — including the qualifying condition listed — is made at the provider’s discretion. We appreciate you taking the time to share your experience and wish you the best moving forward.
The online process was so easy and doing the forms online was very helpful. I was able to schedule my doctor's appointment the same day. The doctor was very nice and provided me with excellent, fast service.
