The appointment was brief (~3mins) and the provider didn’t ask any questions. He approved my recommendation but did not provide any guidance or information at the end. He stated that my recommendation would be uploaded within 24 hours. No additional information was provided. I was told that my name was not on CURT when I called the dispensary 48 hours later.

Elevate made getting my medical card and easy chore. I just followed the instructions, filled out the forms and scheduled a appointment. It all went well. I am now a Mo. Med Card holder.
