I took the paperwork to a dispensary and they said I would need to go to some government office, and no one actually knew what I was supposed to do with the paperwork.

Your email was a survey about a recent purchase. I haven’t had any recent purchases. Update: I have since found out that the survey was about my initial signing up process not recent purchases. The signup and interview with the Dr. went quite well.
