Hi Jayme! Thank you for taking the time to leave us feedback. I see that we have been attempting to reach you as we are missing some required information so that the provider can upload your certification to the state. Once the certification has been uploaded, you will be able to complete the state application so that the card can be issued. I will be asking someone from our Escalations team to reach out to assist.
