Complaint Regarding Mishandling of Prior Authorization and Appeal. I am writing to formally complain about the way my prior authorization request and subsequent appeal were handled by your medical office. Your office submitted a prior authorization request to my medical insurance company, but the request was denied because not all of the required medical documentation was submitted. It took approximately four days for your office to review the situation and determine what additional documentation was required. During this time, I called your office several times and specifically informed your staff that the insurance company had not received all of the necessary documents. Despite my repeated calls and concerns, I received messages and phone calls from your office telling me that I already had an approval and that everything had been taken care of. I also specifically requested that a manager call me to discuss this situation. However, despite my request, no manager ever called me back and when I asked for the name of the manager the person I was speaking with hung up on me. Eventually, your office submitted an appeal. Unfortunately, the appeal was also denied. According to the insurance company, the appeal did not adequately explain why the procedure was medically necessary. It also did not provide sufficient information about whether any non-surgical treatment had previously been performed for my vein condition and, if so, what treatment was provided and whether it was effective. I am very concerned that the denial may have been caused, at least in part, by incomplete or insufficient documentation submitted by your office. I repeatedly contacted your office and clearly communicated that the insurance company was missing documents, but I was reassured that I had already received approval. After the appeal was denied, instead of receiving a call from a manager to discuss the situation and how to resolve the insurance denial, I received two messages asking me to call the office to discuss other payment options. I find this extremely concerning and frustrating. My priority is to receive medically necessary treatment and to have my insurance authorization handled correctly. I do not believe I should be directed toward discussing alternative payment arrangements before the medical office has properly addressed the documentation issues that led to the insurance denial. This situation has caused unnecessary delays, stress, and uncertainty regarding my medical treatment. I found this behavior very unprofessional and inappropriate. Sincerely Oleg Kostiuk Venice FL

Stay away from this company! My wife has been treated there for her veins, was told only a $30.00 copay per visit. All treatments are pre approved thru insurance before all procedures. Now we have a bill due for each procedure, and can't get an answer as to what the charges are! When you call to get an answer, the call center cannot connect you to billing! They can only take the complaints and forward them to billing. This is total nonsense!! And then you want to stop the auto pay, and they cannot even do that! TOTALLY USELESS!! Our only option is to cancel the card, and have a new one issued. On top of all of this, my wife has not seen ANY IMPROVEMENT with her veins. This company is a total scam. STAY AWAY!

I have been a patient for about 2 years. Have no problem with Dr Harvey or Dr Ruth. The Ultra Sound techs have both been great as well as has Dena, the medical assistant that worked with me last week. I have bad knees and venous stasis as well the vein problems. I have worn my own very loose, stretchy shorts for my procedures because it is difficult to change into the paper shorts. Never was a problem until Monday. Not sure what her name is, she didn't introduce herself, but she was rude and condescending about my shorts and she didn't like my compression sleeves either. (They go from my ankle to below my knee.) There was no good reason for the hostility she projected. Hopefully, I won't need any further treatments or I will see about going to Naperville.

Extremely Dishonest Business Practices and Financial Misleading I am writing this review to warn others about the incredibly unprofessional and deceptive financial practices at United Vein & Vascular. What I experienced is nothing short of a "bait-and-switch" that has left me with an unexpected, massive bill and zero accountability from their office. After waiting three weeks for this office to coordinate with my insurance company, I was finally notified that my procedure was approved. When I arrived, the only cost disclosed to me was $16 per pair of compression socks. I proceeded with the treatment under the explicit impression that the insurance coverage was settled and everything was in order. At my follow-up appointment, I specifically asked the front desk if there were any outstanding issues or balances. I was told, "Yes, everything is good." Imagine my shock when, the following week, I received a bill for $1,000 out-of-pocket. This is absolutely unacceptable. The issue here is not that I am unaware of how insurance or copays work; the issue is that this office failed to perform the most basic duty of a medical provider: transparency. They had three weeks during the approval process, the day of the procedure, and the follow-up appointment to provide a clear estimate of my patient responsibility. They never disclosed these costs, nor did they provide me with the option to decide if I could afford this procedure or if I needed to wait until I had the funds. By failing to provide a pre-operation cost estimate despite having multiple opportunities to do so, they robbed me of the ability to make an informed financial decision. This is not just poor communication; it is predatory. I feel completely misled and taken advantage of. If a medical office is unable or unwilling to be transparent about costs until after the services are rendered, they have failed as professionals. I am demanding that they take ownership of this failure and waive these surprise charges. If you are considering treatment here, be warned: they will hide the true cost from you until they have already billed you.

The Doctor told me point blank, ‘Medicare will cover the cost of treatments’. After 5 treatments (both legs), I received a bill of nearly $2,000. I am elderly, and live on a fixed income. I was never told there could be out of pocket costs. The doctor acted surprised when I told her that I would not be returning for an additional 3 treatments, because she neglected to caution me on possible patient charges. Recently I went to a vein surgeon who mentioned he has seen several unhappy patients from the United Vein and Vascular center. It was his professional observation that my right leg never needed treatment, a likely reason why I ended up having to pay for treatments on that leg, insurance would not cover it. Patients beware of the hype if you visit this vein clinic. Their administrative and billing offices are located out of state.

DO NOT GO HERE! I chose UVVC out of convenience, and I only needed treatment in my left leg. I was treated in both legs (precautionary) over 5 visits, and my right leg now has a deep vein thrombosis as a result. This was life threatening. 2 months later, and 3 follow-up visits later, the right leg swelling is actually worse than before, and I've been on Eliquis ever since. Between the procedures and follow-ups, the Richardson clinic has entirely new staff including the physician who performed the procedure. Shady.

I had two persons called within a what of minutes; one scheduled and confirmed appointment, the other called for the same insure and Name information just to cancel the appointment. James-Georgia
