SportSlip provides physician-reviewed sports and camp physical clearances for kids ages 5–18 — entirely online, with no appointment or waiting room. Parents complete a structured screening built on the American Heart Association's 14-element cardiac history and the American Academy of Pediatrics' preparticipation evaluation (PPE-5), submit vitals and a short guided movement video, and upload the actual form their school, camp, or league requires.Every submission is personally reviewed and signed by Dr. Adam Z. Kawalek, MD, a board-certified internal medicine physician licensed in 29 U.S. states (NPI 1326223306) — not an algorithm, not a template. Most families receive their signed clearance within 24 hours. Clearances are valid for 12 months and cover every sport and season. If our screening finds something that needs in-person care, we say so — you get a referral letter and a refund, not a rubber stamp.SportSlip is HIPAA-compliant and a service of SickSlip Inc.

Thank you for making this year’s sports physical seamless! No appointment needed, no waiting in urgent care because you couldn’t get an appt. with your regular PCP. Follow the simple instructions, pay the fee, and upload the forms needed by the school. Signed forms were returned via email within 24 hours.

I needed a sports clearance for my son. This platform was very easy to navigate from my phone. The doctor was very prompt. Will definitely use it again.

My Son needed a sports Physical for high school wrestling, his primary doctor was unable to see him before the season starts so I did a quick search online and found Dr. Kawalek, answered the questions online, uploaded my sons school physical form and had everything done and turned in within 24 hours. Very quick, easy and convenient.

Thank you, Jeremy! We’re glad your son’s form was done and turned in so quickly, and we’ll pass the kind words to Dr Adam Z. Kawalek. Good luck to your wrestler this season!
SportSlip online services was wonderful and efficient especially being done in the comfort of your own home, and with the new school year starting and sports requiring a sports physical. This experience was easy on the teen and the parent. Same day service and results would allow the school of choice to be emailed the results of the sports physical, and the price was fair and reasonable. I’d recommend this route for other parents who have students meeting sports physicals.

Thank you, Luana! Easy on the teen and easy on the parent is exactly what we hope for, and we appreciate you recommending us to other families. Wishing your athlete a great season!
The process was easy enough unfortunately the school didn’t accept it so I had to go pay for an additional physical

We're truly sorry this happened. Thank you for telling us, even though we wish we'd heard it from you directly first. Your review is genuinely the first we learned of your school's decision, and we would have wanted the chance to help: when a school has questions about one of our clearances, we contact them directly -- verifying the physician's signature and credentials, answering their questions, and correcting the form at no charge if something needs to change. More importantly, you shouldn't be out of pocket twice. If you ended up paying for another physical, we'd like to refund your original visit. Please email us at [email protected] with the name on your order, and we'll take care of it right away. We'd also be grateful to know what your school's reason was, as every detail helps us make sure this doesn't happen to another family. We're sorry the ending didn't match how easy the beginning felt. We'd like to fix that.
Dr. Kawalek, was very thorough, and made the experience very easy and effective.

Thank you! Thorough and easy is exactly the combination we're going for. We'll pass the kind words to Dr. Kawalek himself, since he reads every screening personally. Wishing your athlete a great season!
We had just moved to a new state a week before the start of school and were jumping through hoops to get the kids enrolled and get records and paperwork together. I contacted Sportslip support to ask a couple questions about the medical forms and they reached back out right away. I booked later that day and had my daughter's sports form filled out and certified within the same day. It was a very straightforward process and I didn't have to leave home when I already have two million things to do :) Great experience, I would definitely recommend.

Moving states the week before school starts is the deep end. So glad we could take one thing off the list. Welcome to the new state, and thank you for the recommendation!
Needed a sports release for my son to attend and participate at the football game on Friday. Completed the SportSlip process the Monday prior (paid the extra $20 for the expedited service) and received the fully compliant docs from SportSlip Tuesday morning! Immediately processed with the school and my son was cleared for the Friday game that same day! TELLING EVERYONE HOW GREAT SPORTSLIP IS. Even if we aren't in a rush, this process was so much easier and less stressful than booking an in person appointment, then driving to the Dr. in town weeks later (if I'm lucky), waiting around etc etc etc. We will only be using SportSlip in the future for all my kiddos.
