Founded by Dr Adam Zvi Kawalek — a board-certified physician trained at Brown University, Mount Sinai, NYU Langone, Cedars-Sinai/UCLA, and Johns Hopkins — SickSlip was built to address a simple inefficiency in healthcare: people with minor illnesses often have to visit a clinic or hospital solely to obtain documentation for work, while already overburdened doctors and nurses spend valuable time handling routine absence paperwork. SickSlip helps eligible patients obtain legitimate medical absence documentation from a licensed physician in their state without leaving home. Each request is reviewed by a licensed physician and, when appropriate, issued as a digitally signed, verifiable document. The platform focuses on documentation and verification rather than diagnosis or treatment.

Thank you for the kind words and the honest feedback! Our notes are issued on the physician's letterhead, just like any other medical document. Each one carries the physician's name, credentials, and license numbers, along with a QR code and Document ID to make verification smooth.
It was incredibly easy and a lifesaver of an option to have. I would highly recommend using this service. Thank you!

I’m happy that I came across this site. It’s hard trying to get an appointment at the drs. When I’m unwell I know I can count on sick slips doctor. There are times when I’m too ill or in pain to even make an appointment in person. I feel relieved knowing that I don’t have to be super stressed about missing work and being able to provide them an excuse for my absence. Whether it’s my mental health or a cold, it is super easy and convenient to have and use this health service provided to the public. I’ve used it a couple times so far and will continue to do so. I have also already recommended to coworkers and friends! Thanks so much.

I was required to present a medical note for taking a shift off for a family member. But because I attempted to get a cover for my shift, they considered it Sick leave abuse. It was exactly what I needed! Thank you!

easy and fast. costs what it should cost to get a doctors note when you're sick but don't need to go to an urgent care or emergency room. can also get the note written to excuse you for more than one day. I chose 3 days off to give my body time to recover from illness and not stress about getting to school.
