Quick and easy. Spent two minutes with someone who was not the physician who signed my application and who did bother to introduce himself. My application had my name and birthdate printed correctly but it had my residential address rather than my mailing address and a phone number that surely was not mine. The area code wasn’t even correct. I do not recommend this organization.
