I am very pleased to recommend PrestoDoctor.com. Everything was done on line and on time - the form, the photo, the doctors interview - all conducted from my laptop at home! The documents were mailed to home and I had a .pdf to use in the interum.

The account and profile were very easy to set up and only took about 5 minutes. You can do everything through your smartphone, take your selfie to go on the card, take a picture of your ID with the phone and upload it immediately, as well as conduct the interview over the phone using your browser. No additional add-ons needed. The doctor was precisely on time, extremely polite and nice, and was very helpful in explaining possible treatments and potential recommended products. All in all a very smooth experience. My only reservation is that later I found out that most similar services face to face costs about half the price, however I did all of this from the comfort of my own home, so I suppose that's a big plus.
