We understand how important it is for patients to be able to access their medication as quickly as possible. The card will be reshipped today. Hi Dennis! Thank you for your feedback and I apologize for any confusion. In New Jersey, telemedicine visits do not have nor require the provider's signature at the bottom. The registry ID and reference number act as a virtual signature. You should be able to proceed with the state application process. Hi Gaylen! Thank you for providing feedback and I apologize for any confusion caused. To be exempt from taxes on your medical cannabis purchases, our providers can sign the county-issued attestation form. After being approved and certified, our providers are happy to complete this for our patients. With the form and the county application, you can be approved for a state-issued card which will exempt you from taxes on medical cannabis purchases. There is no additional fee for our provider to complete the form and we would be happy to assist with this. Our escalation specialist will be reaching out to offer assistance.
