Hello Richard! Thank you for taking the time to leave us feedback. I sincerely apologize for any frustration caused. Our providers use the intake form to gain more information about your conditions, symptoms, and treatments. The state of Minnesota does require patients to do a self-evaluation so that the pharmacist in your local dispensary can have an understanding of what you are seeking treatment for. They aren't able to see the intake form filled out for the provider, nor is there a way to override their request for the self evaluation. I understand how frustrating that can be.
