Hi Rickey! Thank you for taking the time to leave us feedback so that we can clarify the process. Leafwell doesn't require a photo of you, rather the state needs your picture. This photo will be used on your medical card to be used in medical dispensaries. I apologize for any frustration this has caused. I will ask our escalations team to reach out to see if there is anything we can do to assist. If you need assistance before we reach out, please call us at (800) 660-9085 or you can email us at [email protected].

Hi Kimberly! I apologize for any confusion or misinformation. I see where you were approved and the certification was sent automatically. I also see where you were working with a member of our support team, who resent the certification for you. The certification has the registry ID and reference number required to apply with the state. The state did do away with the $10 state application fee as of 03/01/2024, which is great news. I will ask our escalation specialist to reach out to see if they can offer any assistance. If you have questions before they reach out, please call us at (800) 660-9085 or email us at [email protected].
