I need help filling application out.

It was nice to get approved and I wasn’t told a head of time that there’s still more to process after getting approved. Overall it would be nice to get a refund back but it’s my loss.

No help for customers with vision disability was not happy with customer service

Hi Charles! Thank you for taking the time to leave us feedback. I sincerely apologize for any frustration caused. From the call notes I reviewed, it looks like assistance is needed to complete the state application and a list of state-manned locations at local health departments for in-person assistance. While we can assist with the Leafwell registration, we are unable to complete the state application for patients. I apologize once again. I still have not received my medical card that I was told I'd get. I paid for the digital copy as well as the physical copy and have yet to receive these cards.

We show the card was mailed on 08/23/2024 and it should be delivered within 10 business days. I apologize if it has not been received. I would give it a zero if i could. I am still awaiting the certification from the dr. i spoke with. I followed the after you need to do. It says still awaiting certification.

You can take my money but when I go to get my card you asked me to retrieve a code that takes two devices to retrieve I don't have two devices

As we just the certifying provider's office, we wouldn't be able to see any purchases made in dispensaries. I apologize for any confusion. In your state, after approval and the certification has been uploaded, patients would then log back into the state site. Once logged back in, you will be able to submit your renewal application to the state. We apologize for any confusion.
