Hi Scott! Thank you for the feedback so that we could try to assist. The card is sent by the state after the certification has been uploaded and the annual payment has been submitted. I see the certification was uploaded a few minutes after approval, so I apologize if the card has not be sent yet. Hi Heather! I appreciate your feedback so that we can work to resolve this for you. We ask all providers to discuss allotment needs or concerns during the consultation. If an allotment concern arises after approval, there is generally a fee associated. I apologize that the provider didn't discuss it during your consultation, and I can assure you I will speak with the provider and his manager regarding this. Once the certification is issued after approval, the new card is issued by the state once the stateside application has been completed and approved.
