FadeMD was fantastic. User friendly and cheapest certification with the highest level of care. Cant beat it. if you don't use fade shame on you.

I had to fill out the application for the state of Michigan on line but it wasn't to hard,and I was approved the same day. I had a few questions,and emailed them and they responded pretty quickly. I was delayed because of a ATT outage that lasted about three hours and still got approved the same day. I got my verification by email that acts as my card until mine comes in the mail in 7-10 days from the state of Michigan.

Great fast service. Paid $40 for expedited service and supposed to get provisional card same day. Nope. Use FadeMD, just don't pay for expedited service. Update: They are having me contact IDPH myself. I questioned what the $40 was for. They said they refunded it. We'll see on my next CC statement.
