Dr. was fine. Prices are high, renewal is double cost of others.

They made a lot of mistakes on my application. I still did not receive my license for MMP and it was because the doctor who whatever typed this up put a lot of characters in between my name, my last name, address and now I got to wait for it team to fix all of it. Thanks a lot

However, we want to clarify the following based on our records: Certification Status: As the doctor has stated, your certification was successfully processed and issued. You are currently registered in the CT registry through December 27, 2025, which confirms your approval. Application Accuracy: Our records indicate that your application information, including your name and address, was entered accurately. Registration Status: You are already registered and able to utilize your medical certification. We apologize for any inconvenience you may have experienced during the application process. The medical conditions were left blank. Although it is optional it is also easy to fill out. When I asked about this I was sent the same form with empty conditions. Fully filled out forms are somewhat expected when dealing with a Docotor and State agencies.

i spoke to the doctor, he said my brother? Emric Clayton was approved for the card. zI was told thru email to check the portal for a document, there was nothing stating what his next step would be, How does Emric Clayton obtain a medical card? My phone number is 214-629~9756 Elma Ponder

I had my phone appointment and was approved and told my papers etc would be sent to the state within three days so I could get my card. I already had my state ID # to receive my card and have heard nothing. Two stars instead of one because the doc on the phone appointment was nice and respectful. What’s the deal? Once I know the status of this issue and get my card, I will change my rating and review. Was I scammed? We’ll see.

The whole process was rushed and mishandled so badly that I had to submit a report to the State of Illinois in order to receive my certificate through Ascend. I was never able to reach a support staff member to explain the application process and the “doctor” had me on the phone for 30 seconds without explaining anything.

Went through the card renewal process and paid the fee but never got my renewal id

Hello Steve, there seems to be some confusion, because as of 3/20/2023, NY State no longer mails out plastic cards to patients that are approved. The paperwork issued by the doctor contains a registry ID that can be shown to the shop alongside a government issued ID so you can buy your medications. This doctor's certificate is available for you in your patient portal. If you need any assistance, let us know by emailing us at [email protected]
I entered my discount code but was still charged full price on my card after further inspection i contacted customer service and still waiting for the refund.
